Your clinic handles the medication. I handle the food.

You still have to get enough protein on a small appetite, get through the nausea weeks, and hold onto your muscle while the weight comes off. That half was never what a prescription was for. I'm Nate, a registered dietitian, and it's the part I take off your plate and your clinic's.

If you're on a GLP-1

Some of this is probably familiar

Eating has become a chore.

Nausea, sulfur burps, constipation, foods you used to like that now turn your stomach, days you have to remind yourself to eat at all. Most of this responds to what you eat and when, not to pushing through it.

You're losing more than fat.

Across nine controlled trials, roughly 30% of the weight lost on these medications is lean mass. Muscle is what holds your strength and your metabolism up, and it does not come back on its own when you stop.

You've wondered what happens when you come off.

In the trial that followed patients after stopping, participants regained about two thirds of what they had lost within a year. That isn't a mark against the medication, which did exactly what it was meant to do. It's that appetite came back and the eating habits underneath it had never been built. That's what the maintenance plan is for, and we start it in month one rather than at the end.

Sources are listed below, in plain sight, because you should be able to check them.

Hi, I'm Nate

The person on the other end of this

I'm a registered dietitian. I've worked in hospitals, in community health, and with collegiate and professional athletes, and all of it taught me the same thing: the same symptom almost never has the same cause in two people.

A lot of the people I work with now started one of these medications, watched it work, and then hit the food half with nothing to go on. That isn't anyone behaving badly. Your prescriber is handling the medicine, and handling it well. This part was simply never on the same page as the prescription. It's the part I do, alongside them.

The work

What I actually do

  • A protein target you can hit on the appetite you actually have
  • Meal structure built around your titration schedule, not a generic template
  • Symptom management: nausea, constipation, reflux, sulfur burps, food aversions
  • An extra check-in on every dose escalation week, because that's when it gets hard
  • Nutrition to protect muscle, matched to what you can realistically train
  • A maintenance plan built before you need it, not after
How it works

Four steps, and the first one is the longest

  1. Apply.

Detailed, and worth the time it takes. Your current dose and titration date, symptoms so far, what you can actually keep down right now, health history, and labs if you have them. I read every answer myself before I build anything.

  2. Enroll and get on a call.

Twenty to thirty minutes, scheduled right after you check out. This is where I catch the things a form cannot, and where your plan actually gets shaped.

  3. Get your plan within 48 business hours.

Built for your dose, your schedule, and your appetite. Not a starter kit.

  4. Check in weekly.

Everything runs through messaging and your app. I review, adjust, and tell you the next step. Dose escalation weeks get an extra touchpoint.

How this fits with the care you're already getting

Your clinic manages your medication, your dose, and your medical care, and that is exactly where those decisions belong. My job is the food half: what you eat, how you handle the side effects, and protecting your muscle while the weight comes off. When something crosses from a food problem into a medical one, I send you straight back to them the same day. Everything I build is meant to make their protocol easier to stay on.

Where these numbers come from

The research, in full

  1. Roughly 30% of the weight lost on GLP-1 therapy is lean mass.

Beavers KM, et al. Obesity 2025;33(2):225-237. Meta-analysis of 9 randomized controlled trials.

  2. About two thirds of lost weight is regained after stopping.

Wilding JPH, et al. Diabetes Obes Metab 2022;24(8):1553-1564. STEP 1 extension.

  3. GI symptoms are common and mostly early. 44.2% nausea, 24.8% vomiting, largely within the first 20 weeks.

STEP 1 trial safety data. Wilding JPH, et al. N Engl J Med 2021;384(11):989-1002.

Questions

Questions people ask before they start

Do I have to be on a GLP-1 to work with you?

No, but this program is built for people who are.

What if I'm barely hungry?

That's the normal starting point, and it's most of the problem worth solving. The plan is built around a small appetite rather than assuming you can eat more.

Will you talk to my prescriber?

If you want me to, yes. If your clinic wants a monthly summary, I'll send one. I never share anything without your say-so.

What happens if I stop the medication?

That's the part most people don't plan for, and it's why maintenance is part of the plan from the start rather than something we get to later.

Is this included in what I pay the clinic?

No, it's separate. Your clinic handles your medication and your medical care, and you pay them for that. Nutrition is mine, and you pay me directly for it.

Do I need labs to start?

No. If you have recent bloodwork, share it. Interpretation is included at this tier.

Do I have to buy supplements?

No. They're optional on every plan.

Can I cancel?

Anytime. No contract, no minimum.

Start with the first four weeks

This is the guide I hand every client in their first month: what to eat while the nausea is bad, how to get protein in on no appetite, and what to expect at each dose step. Tell me where to send it and it's yours to start on today.

The medication is doing its job. This is the other half.

Apply, share the full picture, and I'll have your plan within 48 business hours of enrolling. Month to month, cancel anytime.
